Physical Health: I acknowledge that I have informed my doula of any medical conditions that may affect the outcomes of my pregnancy and the services provided. This information is accurate and true to the best of my knowledge.
Non-Medical Services: I understand that the doula is not a medical professional. I understand that the doula will not be able to provide medical services, such as medical examinations and diagnosis, monitoring of the baby’s health, or prescribing medications. I agree to consult with my healthcare provider for medical concerns and decisions.
Emergency Situations: I understand that in the event of an emergency, the Doula will defer to the medical professionals present and follow the protocols and instructions provided to ensure the safety of me and my baby.
Release of Liability: I hereby release, discharge, and hold harmless Embracing Me LLC, its owners, employees, agents, and representatives from any and all claims, liabilities, demands, actions, or causes of action related to any loss, damage, or injury, including death, that may be sustained by me due to the provided services, except in cases of gross negligence or willful misconduct.
Indemnification: I agree to indemnify and hold harmless Embracing Me LLC against any and all claims, liabilities, actions, costs, losses, damages, or injuries arising out of the provided doula services, except in cases of gross negligence or willful misconduct.
